Our Community Health Improvement Plan (CHIP) is a coordinated plan of action to respond to the health needs of the community.
The purpose of Lake County’s Community Health Improvement Plan is to:
Identify our community health priorities
Monitor our progress in achieving these outcomes
Improve our community’s health
Focus our attention and resources on strategies to affect positive health outcomes
The Live Well Lake County Committee is made up of representatives from a variety of community based organizations, government agencies, health care systems, and academic institutions. The purpose of the committee is to guide the community health assessment process, prioritize community issues, and collaborate with one another and with stakeholders outside of the committee to take action and improve the overall health and well being of residents in Lake County.
